Chapter Forms
Each of the forms listed in the near-left panel is presented in Microsoft Word format (except for the IRS Tax Filing Procedure, which is presented in Adobe Acrobat format). Depending on the form, it may be mailed, emailed (rjdryden@msn.com) or faxed (503-297-3694) to the Alpha Pi Mu National Office after obtaining the necessary signatures. The one exception is the IRS Tax form (Form 990-N/e-Postcard) which must be submitted online to the IRS. The initiation certificates and keys will be shipped to your chapter after the initiation payment is received.
The due date and eligible modes of transmission for each form are provided in the following table:
